Pre-congress Workshop: Low-cost Interventions to Address Disability Worldwide

Description: This theoretical–practical workshop will provide training in rational and resource-conscious lower limb prosthetic prescription, fabrication of low-cost plaster standing frames and molded seating for children with disabilities, and long-term health maintenance and economic independence strategies for persons with spinal cord injuries.
Section I (60 minutes): Lecture on the theoretical aspects necessary to appropriately carry out the practical component of the workshop, and presentation of other low-cost interventions such as hyaluronidase injections for the management of spasticity-associated stiffness, early neurodevelopmental assessment, analgesic procedures such as nerve blocks and infiltrations, and assessment and prescription of therapeutic exercise, among others.
Section II: Rational and Resource-Conscious Lower Limb Prosthetic Prescription (60 minutes)
Introduction and rationale: the objective of this workshop is to standardize prosthetic prescription criteria through a multidisciplinary approach. The correct selection of components depends not only on available technology but also on a rigorous clinical evaluation of the amputee's activity level and functional needs.
Structure: To maximize hands-on learning, the workshop is divided into three 15–18-minute stations, allowing for a dynamic rotation (rotate clockwise) of attendees.
Station 1: Clinical and Functional Assessment

Objective: To train attendees in identifying functional levels (K-Levels) and taking critical measurements.
• Content:
o Review of the mobility scale (K0 to K4).
o Application of functional tests (e.g., Timed Up and Go or Amputee Mobility Predictor and others).
o Residual limb measurement protocol: circumferences, lengths, and skin/tissue status.
Station 2: Interfaces and Suspension (Sockets)

Objective: To differentiate socket designs based on the level of amputation and residual limb characteristics.
• Content:
o Transtibial: Patellar Tendon Bearing (PTB) vs. Total Surface Bearing (TSB) / vacuum sockets.
o Transfemoral: differences between ischial containment and sub-ischial sockets.
o Suspension systems: pin, suction, and active vacuum.
Station 3: Prosthetic Components (Knees and Feet)

Objective: To select the appropriate technology based on the user's biomechanical profile.
• Content:
o Knees: mechanical (monocentric/polycentric) vs. microprocessor controlled. Focus on stance phase stability and swing phase control.
o Prosthetic feet: dynamic response (carbon fiber), multiaxial feet, and other options for various terrains.
Requirements for development (logistics)
To ensure the success of the activity within the congress framework, the following materials and resources are required:
• Audiovisual Equipment: A digital projector and a suitable projection surface/screen are required. The space must be configured to allow the use of personal computers (laptops) for presentations. A visible timer for rotations.
• Prosthetic Components: A variety of prosthetic feet and knees for hands-on demonstration. If available, sample sockets (transtibial and transfemoral) are also requested.
• Exhibitor Collaboration: Since these components are typically showcased at the commercial stands, we propose a collaboration with the exhibitors present at the congress. We would appreciate the organization's assistance in facilitating the temporary loan of these elements from the available stands for the duration of the workshop.
Section III: Fabrication of Low-Cost Plaster Standing Frames and Molded Seating for Children with Disabilities (60 minutes)
Station 1: Theoretical Presentation of Postural Adaptation Programs for Children with Disabilities (20 minutes)
• This presentation will address postural management programs for pediatric patients, focusing specifically on those with cerebral palsy. We will define the objectives of this intervention, the optimal timing for implementation, and the clinical benefits. Furthermore, we will explain the use of standing frames and molded seating systems, highlighting their functional importance and contribution to improving the child's quality of life.
Station 2: Demonstration on Fabricating Low-Cost Plaster Standing Frames and Molded Seating (10 minutes)
• This session will explain the step-by-step process for fabricating low-cost standing frames and seating systems using plaster bandages.
Station 3: Low-Cost Fabrication of Plaster Standing Frames and Molded Seating Using Dolls (30 minutes)
• Participants will have the opportunity to create their own standing frame or seating system using a doll. This hands-on activity is designed to help them develop their skills in handling plaster bandages.
Section IV: Long-Term Health Maintenance and Economic Independence Strategies for Persons with Spinal Cord Injuries (60 minutes)
Methodology: Simultaneous/Rotating workstations.
Introduction and rationale: The objective is to explore and to disseminate the sustainable low-cost model/settings multidisciplinary care of patients/person lived experience with SCI in the community by the digital technology and provide vocational training for the persons with physical disabilities for their economic independence.
Station 1: Digital innovation (telerehabilitation). 20 minutes of workshop/presentation and 10 minutes of Q&A.
Station 2: Economic independence (vocational rehabilitation). 20 minutes of workshop/presentation and 10 minutes of Q&A.
