Hana Azizi, MD, MPH is the David Gurewitsch Assistant Professor of Clinical
Rehabilitation Medicine at Columbia University College of Physicians and Surgeons. She serves as
the Director of Pediatric Physical Medicine and Rehabilitation and the Fellowship Program Director of
Pediatric Rehabilitation Medicine in the Department of Rehabilitation and Regenerative Medicine. Dr.
Azizi earned her Doctor of Medicine (MD) from Shiraz University of Medical Sciences and later obtained
a Master of Public Health (MPH) from Tehran University of Medical Sciences. She completed
her Physical Medicine and Rehabilitation Residency at Montefiore Medical Center, Albert Einstein
College of Medicine, and a fellowship in Pediatric Rehabilitation Medicine at NYU Langone Health, Rusk
Rehab. Dr. Azizi has extensive experience in treating children and adults with cerebral palsy, complex
pediatric rehabilitation conditions and teaching ultrasound.
