Objectives : : Osteoarthritis is among the most prevalent musculoskeletal disorders, causing pain, loss of motion, and functional impairment. Given the limited evidence on effective intra-articular therapies, this study aimed to compare the efficacy of intra-articular collagen against corticosteroid injections in improving pain, hand function, and hand strength in patients with first carpometacarpal joint (CMC-1) osteoarthritis

Design: : In this double-blind randomized controlled trial, patients with CMC-1 osteoarthritis referred to Shahid Modarres Hospital, Tehran, Iran, between April and November 2024, were enrolled. Demographic and clinical data, including pain intensity, hand function, and hand strength, were assessed at baseline, one month, and three months after intervention

Results: Fifty patients (27 females; mean age, 57.12 ± 9.47 years and 23 male; mean age, 59±10.1 ) were randomly assigned to receive either a collagen or corticosteroid injection. At one-month post-intervention, corticosteroid injection produced a greater reduction in pain intensity (2.24 ± 1.36 vs. 4.88 ± 1.48; P < 0.001) and improvement in hand function (1.60 ± 1.47 vs. 5.28 ± 3.87; P < 0.001). However, by three months, pain intensity was comparable between subgroups, while the collagen subgroup demonstrated superior improvement in hand function (1.80 ± 1.73 vs. 3.32 ± 2.09; P < 0.001). Hand strength improved similarly in both subgroups over the study period

Conclusion: Both corticosteroid and collagen injections significantly improved pain, hand function, and strength in patients with CMC-1 osteoarthritis. Corticosteroid injections provided greater short-term benefits, whereas collagen injections yielded comparable pain relief and superior functional improvement at three months
