Objectives : Data comparing percentage-predicted VO2peak recovery across surgical methods in NSCLC remain limited. This study compared % predicted VO2peak changes between lobectomy and sublobar resection and identified factors influencing short-term postoperative recovery.

Design: This retrospective cohort study analyzed 194 NSCLC patients who underwent lobectomy, segmentectomy or wedge resection between December 2020 and July 2024. Of 640 screened, patients with baseline CPET and follow-up data at one and/or three months were included. The primary outcome was % predicted VO2peak from CPET. A linear mixed-effects model assessed the interaction between surgical method and time, adjusting for baseline covariates that differed significantly between groups. Additionally, multivariable linear regression identified independent predictors of % predicted VO2peak at three months postoperatively. Groups were generally comparable in age, sex, BMI, pulmonary function, and CPET indices, with significant differences in tumor location, histologic type, stage, size, and specimen weight.

Results: Changes in % predicted VO2peak over Time According to Operation Extent and Clinical Factors Influencing Recovery were showen. The mixed-effects model showed a significant time-by-group interaction, indicating distinct recovery trajectories. In lobectomy group, % predicted VO2peak declined from 101.55 ± 1.67% preoperatively to 81.24 ± 1.56% at one month, followed by partial recovery to 87.80 ± 1.81% at three months. Segmentectomy or wedge resection resection group showed a smaller reduction from 102.18 ± 3.28% to 88.31 ± 3.05%, improving to 95.29 ± 3.49%. At 3 months postoperatively, higher preoperative predicted V̇O₂peak%, higher preoperative FEV₁%, and female sex were independently associated with better recovery of postoperative % predicted VO2peak, whereas presence of postoperative complications was significantly associated with poorer recovery.

Conclusion: Lobectomy shows a greater initial reduction and slower partial recovery in % predicted VO2peak than sublobar resection. Higher preoperative cardiopulmonary function and female sex were associated with better postoperative recovery of % predicted VO2peak, while postoperative complications negatively affected recovery.
