1000 Section - Functional evaluation following enzymatic escarolysis and scar treatment outcomes: a case-control study

Objectives : Burn-related edema results from the acute inflammatory response to thermal injury. Enzymatic debridement reduces edema by removing necrotic tissue, and relieving compression of underlying tissues. This leads to early improvement in joint mobility and long-term functional autonomy. Furthermore, the fibroblast hyperproliferation and development of hypertrophic and retractile scars are limited. The study analyzes the efficacy of non-invasive treatment with pharmacological (Sameplast gel) and physical (silicone plaster) effects for prevention and treatment of scarring hypertrophy in burn patients.

Design:

This was a retrospective, observational, two-arm case–control study. Patients were enrolled from January 2022 and followed in a dedicated scar clinic. All patients underwent enzymatic debridement within 72 hours of injury. Forty days after wound healing, treatment was initiated in hypertrophic areas and continued for six months. Subjects were divided into 2 groups (n=10): A, the protocol (gel applied daily and plaster nightly), and B, the control (emollient moisturizer). Pressure garments were used constantly. Scar outcomes were assessed using the Vancouver Scar Scale (VSS) and Patient and Observer Scar Assessment Scale (POSAS) at 3, 6, and 12 months.

Results:

At baseline, VSS and POSAS scores were comparable between groups. At 12 months, Group A showed greater improvement: mean VSS scores 2.4 versus 3.0 in Group B. POSAS patient and observer scores were also lower in Group A (3.6 and 3.4) compared to Group B (4.2 and 3.8). At 6 and 12 months, scars progression and Quality of Life showed greater improvement for group A.



Conclusion:

Limitations include small study group, and time-sensitive for protocol implementation. The combined use of Sameplast gel and plaster evidences overall improvement in scar quality, reducing functional limitations. This could be considered a surgical-rehabilitative approach to be included in scar treatment protocols.
